Waiver & Release of Liability

In consideration of being allowed to participate in the Aboriginal Thunder Hockey Championships hockey tournament and related events and activities, the undersigned, on behalf of their attending team, acknowledges and agrees that the Aboriginal Thunder Hockey Championships, its officers, directors, officials, referees, coaches, agents, employees, other participants, arena officials and owners, and advertisers shall not be held responsible for any accident, injury, or loss however caused, and hereby releases and holds harmless all of the foregoing with respect to any and all injury, disability, or loss to person or property, whether caused by the negligence of the released parties or otherwise.

The Team Representative who signs and/or prints their name on this waiver further acknowledges and states that the team they represent carries fully paid‑up valid liability insurance with a minimum coverage of $2,000,000, and that the team (including players, parents/guardians, coaches, manager, trainer, and other staff) also carries fully paid‑up medical and accident insurance.

The Team Representative signing below also acknowledges that unforeseen incidents, including but not limited to COVID‑19 or other unknown circumstances, may arise. In the event that the Aboriginal Thunder Hockey Championships tournament must be postponed, entry fees are non‑refundable; however, a credit toward the following year’s tournament will be issued.
